Abstract:

Background & Objective: the present study was undertaken to ascertain the incidence of early, advanced cancer Cervix and its recurrence and role of chemotherapy in locally advanced cancer Cervix.Material and Methods: The present study was conducted in the Departments of Obstetrics & Gynaecology, Pathology and Radiotherapy and the specimens examined in the Department of Pathology of Jawaharlal Nehru Medical College, Aligarh Muslim University, Aligarh, India from July 2009 to July 2010. One hundred histologically confirmed Carcinoma Cervix cases were divided into two groups. I: Study group of cases receiving different types of chemotherapy and II: Control group comprised of cases on pre and postoperative radiotherapy and surgery. Patients were staged and locally advanced inoperable cases (Stage IB-IVA) were given different types of chemotherapy. Thereafter subjective and objective assessment was made according to World Health Organization (WHO) criteria of tumor response. Results: Majority of cases (60.0%) presented in stage IIB of disease by International Federation of Gynecology and Obstetrics (FIGO) classification. Commonest symptom was postmenopausal vaginal bleeding, 70.0% cases.68.0% & 56.0% cases showed complete response (CR) on chemotherapy and radiotherapy respectively. Stage IIB patients showed the best response with CR in 70.0% & partial response (PR) in 16.7% cases. Colposcopic evaluation on 12 -18 months follow up showed 08 to be disease free, with decrease in size of tumor. Conclusion: Chemotherapy is an effective mode of therapy and can be considered as an adjunct to surgery or radiotherapy to improve the overall survival of cancer Cervix patients.

Abstract:

Background: Small cell Carcinoma of Cervix (SCCC) is a rare disease. SCCC is highly invasive and prone to distant metastatic spread and lymph node involvement. Here we aim to present a patient and her treatment. Case Presentation: We report 47-year-old patient with history of breast cancer manifesting with abnormal vaginal bleeding diagnosed with SCCC. Patient underwent radical hysterectomy and bilateral salpingo-oophorectomy. Then, she received adjuvant chemoradiation postoperatively. Conclusion: Small cell Carcinoma of Cervix is an aggressive form of cervical cancer with poor prognosis. Optimal treatment remains unsettled.

Abstract:

The presence of cardiac metastasis from cervical Carcinoma is extremely rare. Most of the cases were diagnosed postmortem. There are a few cases of premortem diagnosis and it is believed that in these cases, the prognosis is extremely poor. We present the only case of premortem diagnosis of cardiac metastasis of more than 500 cases of cervical Carcinoma in our center during the last 10 years. We present a 59 -year - old woman with multiple ventricular metastasis from cervical Carcinoma, 2.5 years after the primary diagnosis. She died 10 days after echocardiographic diagnosis of multiple metastatic lesions in heart, following a massive DIc.The rapidly progressive DIC, and death, in the presenting case, confirms the previous finding of poor prognosis of patients ( pts.) with premortem diagnosis of cardiac metastasis of cervical Carcinoma

Abstract:

Background: Carcinoma of the uterine Cervix is the sixth most common malignant neoplasm in women. Early stage diagnosis of uterine Cervix Carcinoma increases the cure rate of disease. Radiotherapy, with or without concurrent chemotherapy, is one of the most effective treatment modality in cervical Carcinoma. After radiotherapy, accurate and regular follow-up results in early diagnosis and effective treatment of recurrence. The aim of this study is the assessment of the pathologic characteristics, setting and indications for postoperative radiotherapy and outcome of patients at the last follow-up. Methods: In this retrospective study, we evaluated 346 cases of cervical Carcinoma had received radiotherapy in radiation oncology department of the Cancer Institute of Imam Khomeini Hospital, Tehran, Iran, from 1995 to 2001. Results: The age of the study group ranged from 26 to 78 years (mean=50.5, SD=11).Of these patients, 30.4% were in the early stage and 69.6% had advanced stage disease. Squamous cell Carcinoma comprised 92.2% of the cases and 6.4% adenoCarcinoma. Radical radiotherapy was the most frequent radiotherapy setting and adjuvant radiotherapy (post-op) was the second. A considerable number of patients did not come back for follow-up, and 43.7% were followed for a short time. Conclusion: Accurate presurgical clinical staging and surgical treatment by an expert gynecologic oncologist can reduce expose the patients to risks of two modality treatments (surgery and radiotherapy). According to our results, patients do not pay enough attention to disease follow-up. An acceptable training plan with emphasis on regular follow-up, is recommended.

Abstract:

Background: Verbascum species showed various pharmacological activities including anti-inflammatory, antitussive, antiulcerogenic, immunomodulatory, antimicrobial, antimalarial, antioxidant, and anticancer activities. Objectives: The aim of this work was to evaluate cytotoxicity of different fractions of Verbascum alceoides, which belongs to this genus. Materials and Methods: Aerial parts of this plant were collected from Doveiseh area in Kordestan province. The plant was extracted using a four-step extraction method with increasing solvent polarity (i. e., hexane, dichloromethane, chloroform-methanol [9: 1], and methanol). The methanol extract was finally separated between water and butanol. Hexane, dichloromethane, chloroformmethanol, butanol, and aqueous partitions were then subjected to cytotoxicity evaluation. Showing the most potent cytotoxic effects, the butanolic partition was further fractionated by medium-performance liquid chromatography and similar eluates were pooled to prepare five final butanolic fractions, named A– E. Results: In vitro cytotoxicity of these fractions against human cervical epithelioid Carcinoma (HeLa) and human umbilical vein endothelial cell (HUVEC) was evaluated using 3-(4, 5-dimethyl-2-thiazolyl)-2, 5-diphenyl-2H-tetrazolium bromide assay. Fractions D, E, and A  showed a significant and dose-dependent inhibition of cell proliferation (half maximal inhibitory concentration [IC50 ] of 30, 39. 8, and 188. 6  µ g/mL, respectively). According to the preliminary thin-layer chromatography analysis, these cytotoxic effects may be mainly due to presence of saponin and flavonoid compounds. Conclusion: Future studies will be aimed to isolate and purify active constituents and investigate the effect of them on more different kinds of cancer  cells.
